BRIEF DESCRIPTION OF THE INSTITUTION
The organisation Servicio Madrileño de Salud (SERMAS) is the administrative and management structure that integrates every public hospital and every public health service of the Madrid Regional Health System. SERMAS, in the FP7 and HORIZON 2020, is the legal representative (beneficiary) of every public hospital of the Madrid Regional Health System. Usually SERMAS delegates authority to the Hospital’s General Director to manage the research project favourably evaluated by the Commission. In this project, SERMAS (beneficiary) is the legal representative of Hospital Clínico San Carlos and delegates authority to Hospital’s General Director to manage research projects. Fundación para la Investigación Biomédica del Hospital Clínico San Carlos (FIB HCSC) is the third party attached to this hospital. Collaboration between SERMAS and FIB HCSC is carried out through a prior collaboration agreement, by means of which the latter handles the financial and administrative aspects of the Hospital’s involvement in research projects, including all issues relating to the employment and payment of additional personnel, purchase of equipment and consumables, etc. The Foundation is situated in the premises of the Hospital. Accordance to it, the EC contribution is directly paid to this Foundation as Third Party, and not to the beneficiary. FIB HCSC is a type of Third parties making their resources available to a beneficiary free of charge- Special case: Foundations, spin-off companies, etc., created in order to manage the administrative tasks of the beneficiary”.
PREVIOUS EXPERIENCE RELEVANT TO THE TASKS
The research team has extensive experience of the geriatric assessment. Well know instruments of cognitive, functional, affective and social assessment of elderly patients. The integration of these assessments is the basis for management of elderly. Longitudinal studies of aging here successfully carried out a healthy elderly population. We have include several national and European projects

The Hospital Clínico San Carlos currently employs more than 5,000 professionals and is the referral center for the training of health of the Complutense University of Madrid together with Gregorio Marañón hospital and October 12. The number of beds is 1,193, 214 outpatient facilities, 30 operating rooms and diagnostic equipment of last generation.
SHORT PROFILE OF KEY STAFF MEMBERS
Pedro Gil Gregorio: Dr. Gil is chief of Geriatric Department of Hospital Clinico San Carlos, professor of Complutense University and president of Spanish Geriatric Society. He leads a multidisciplinary research team on the process of aging in various systems including the kidney. He has over 150 publications in international journals and has participated in clinical trials as well in funding grants. He has extensive experience in comprehensive geriatric assessment and management of comorbidity and chronicity.
Patricia Alonso Fernandez: Dr Alonso is a geriatrician of Hospital Clinico San Carlos and she leads the Osteoporotic Elderly Program and she is working in a centenarian study and inmunogerontolgy.
Jose A. Herrero Calvo: Dr Herrero is a nephrologist doctor who works in the consultation of advanced kidney disease (CKD) in which there is a high prevalence of elderly patients. He is a member of the Technical Committee of the Madrid Registry of renal replacement therapy patients.
Fernando Tornero Molina: Dr Tornero is a nephrologist doctor who works in the consultation of advanced kidney disease (CKD) in which there is a high prevalence of elderly patients. He has a great and long experience in programs based on the attention to elderly people with CKD and high blood pressure. He is a member of the Technical Committee of the Madrid Registry of renal replacement therapy patients.
Raquel Yubero Pancorbo: Professor Yubero Pancorbo is a neuropsychologist with an extensive experience in the realization of research project. She is a professor of the Complutense and Comillas University. She has been working at the Department of Geriatrics for over ten years.
